Records check authorization
In order to provide guests with access to controlled areas of the jail and the full day-in-the-life experience of a Corrections Deputy, there are some security measures in place that must be followed. Each guest needs to complete and submit the electronic Records Check Authorization form. Please know that this is standard practice for most jail facilities.
- This confidential form is used to establish your identity and verify the existence of any relevant criminal history records.
- All demographic and identifying fields are required to establish your identity. Your Social Security Number (SSN) is not mandatory, but is helpful in keeping records accurate in the event other people have the same name and date of birth. Failure to disclose your SSN may prevent or delay the processing of your records check for the event.
- Please list ALL arrests, incarcerations, and/or convictions in the space provided. If no arrest, incarceration and/or convictions exist, please write “NONE” in the first space provided.
You may complete, sign, date and submit the form electronically; it does not need to be printed. Please submit the Records Check Authorization form as soon as possible to allow time for processing.
